Each of the forms templates above contains the basic information that a club needs to perform each task. Most Clubs will want to customize, add features and expand options on these forms.

Here are just a few things you can add to these basic forms to customize them.

• Add an authorize.net feed to take payments,  this is mandatory for taking payments and must be set up each time you create a new form or copy a form
• Limits to the form or the field.
• Use the form to place a tag on a members account to be used for tracking or future pricing
• Send a custom notification to the admin and Member when a form is filled out
• Add a Service Fee to a form

Example #1:  The “Board Election” or the “Simple Survey”, these 2 forms were built using industry standards.  In the case of the Board Election  the selection process uses ‘radio button’ so that only 1 person can be selected for each of the candidate positions. Your pool admin will need to update all of the names and do any further customization that they feel is need. The Pool Admin (if experienced) may even want to change the ‘radio buttons’ to ‘check boxes’ and the like.

Example #2: The Lesson Sign Up form has most of the basic  information that a club needs to set up a lesson. The form can be used for swim lessons, yoga, tennis and the like. You can then customize this form by adding all of the items you need to make it fit your needs (such as birth dates, ages skill level etc.). In addition this would be a good form to add a custom notification to this way the parent can look at their email and see which classes they signed up to take their children to.
