Download and Install Instructions:
- Click the orange “Get This Form Template” button to download the form file and save it to your hard drive.
- In your website, from the menu on the left-hand side of the screen, go to Forms > Import/Export.
- On the Import/Export Forms page, Select Import Forms.
- Choose the form file that you downloaded and import the form file.
- You are done! You can always make additional modifications to the form to accommodate any specific needs.
